Managing the Current IEP Folder

Each student in special education must have a green folder or green-tabbed folder labeled “Current IEP Folder” that is part of the student cumulative folder. The Current IEP Folder should be labeled with the student’s name (last, first) and date of birth. The Current IEP Folder should contain the following information:

1. Access log to document approved individuals who review the folder. The special education licensed staff in the school have the authority to access the record without signing the Access Log. All other entries should have a date of access, signature of the individual and the purpose for accessing the record.

2. Chronological log for listing due process activities, especially documentation of contacts with parents or others involved. The contact log on Campus should be printed on a regular basis and placed in the special educaiton file. 

3. Due Process forms within the calendar year of the current IEP including:

  • Notice of Special Education Services signed by the parent or indicating the 14 calendar day rule.
  • IEP/IIIP written within the calendar year.
  • Notice of a Team Meeting for the current IEP/IFSP/IIIP.
  • Notice of Educational Evaluation/Reevaluation Plan signed by the parent for an initial evaluation or 14 calendar day rule if a reevaluation.

  • Evaluation/Reevaluation Summary Report completed within the last 3 years.

  • Notice of a Team Meeting to determine eligibility for special education.

  • Progress Reports when written reports are completed.

  • Test protocols used for the evaluation or latest 3-year reevaluation.

    Records kept in the Current IEP Folder should relate only to the Current IEP. All old or other records should be placed in the student’s cumulative file. Copies of the records supporting the initial eligibility evaluation and IEP must be retained in the cumulative file and eventually microfilmed when the cumulative folder is destroyed.
