Coordinated Plan
The Coordinated is a plan that can be developed for students with disabilities who receive services from public agencies.  Typical public agencies that might be involved with students with disabilities include Ramsey County Human Services, Head Start, Child Protection, Corrections, etc.  The purpose of the Coordinated Plan is to integrate all public services into one plan to serve students and families more effectively and efficiently.  The development of a Coordinated Plan is optional for parents.  The parent may elect to continue with an IEP and the separate plans that the public agency develops or the parent may elect to integrate the IEP with the other separate plans into a Coordinated Plan.

The IEP manager of any student with an IEP who also receives service(s) from a public agency should offer the option of a Coordinated Plan to the parent.  The IEP Manager should have the parent sign the "Coordinated Planning Process for Ramsey Co. Residents" form.  This form can be downloaded from this website.

When developing a Coordinated Plan, the IEP Manager is responsible to set up the meetings and notify the coordinated plan members.  The timeline and due process requirements of the Coordinated Plan are the same as the timeline and due process requirements of the IEP (e.g., required team membership presesnt, written annually, at least 7 days notice for an IEP meeting, etc.).
